Baseline Educational Assessment 

Baseline assessment principles for moving from Children’s Services to Adult Services

The baseline assessment provided by the team consists of a comprehensive analysis of each young person’s skills, abilities, support needs and background, with the aim of deciding where support is best put in place.

What We Cover

  • Experience of school and qualifications attained (if any).
  • Prior experiences, skills, likes/dislikes etc
  • Literacy and Numeracy skills 
  • Current support being received from LA, family, Social Workers e
  • Current SEN status
  • Current housing status
  • Financial awareness
  • Independent living skills
  • Nutrition and wellbeing 
  • Independent travel skills
  • Current employment/education status
  • Long term goals & aspiration

Support Resolution Core Support

We assess several areas, all of which sit under our Core Support - 

R         Right Support 1-1 including night, (Right Care, Right Culture)          

E          Education, Employment & Career Support

S          Safeguarding and Deprivation of Liberty Safeguards “Continuous Supervision & Control”

O         Observation and Management of Behaviors and Incidents

L          Life & Domestic Skills including support with budgeting, and benefit applications (PIP/UC) 

U         Understanding Social Norms and appropriateness 

T          Time management, including supporting with or the administration of medication

I           Inclusion in the community and support within family setting

O         Observation and Management of Health & Wellbeing 

N         New hobbies, interests and things to look forward to
